Experience with a Diabetes clinic is an added advantage

Job Responsibilities :
Coordinate with doctors, floor executives and others concerned to manage patient navigation
Counsel patients; inform & educate them on the program and guide patients on their participation
Coordinate with care manager on delivery of services to patients, and coordinate on delivery of services across program calendar
Generate and provide doctors with reports (such as “Risk Stratification” & “HRA”); when required coordinate with them on patient enrollments
Coordinating with doctors on patient referrals; monitor and aid patient navigation on aspects such as foot and eye screening
Patient enrollment, which includes capturing their demographics, QOL, investigation values into Promis and also training patients on the use of glucometer
Coordinate with the Call Centre Executives, doctor and hospital team on patient related activities with the help of the software
Coordinate with those concerned on patient referrals for the program
